LEARNING DISABILITIES

Support for young adults and adults with learning disabilities, including those with autism, physical disabilities and complex needs.

Glenholme provides support for people with learning disabilities in residential services, supported living and through out-reach. In all locations the focus is on promoting independence and engaging as fully as possible with the wider community. Care plans are person centered and flexible and we work closely with the individual, their family, friends and professionals to ensure that we are delivering the best support possible.

Support for individuals with learning disabilities and complex needs.

Glenholme has a strong track record [delete in] supporting adults and young adults with learning disabilities and autism including:

  • People who have behaviours that challenge
  • People with physical disabilities and complex physical needs (e.g. peg feeding, Epilepsy, Cerebral Palsy)
  • People with learning disabilities and mental ill health, including those that self-harm
  • People with additional disabilities such as communication difficulties and sensory impairments and other complex needs
  • People with forensic histories

MENTAL HEALTH

Support for adults with mental health problems, including those with, learning disabilities, complex needs, forensic histories and addictions.
Our services support those with mental health conditions. We recognise that these are often accompanied by other diagnoses and we are experienced at providing genuinely person centred support which bridges traditional medical divides, thereby ensuring the greatest chance of recovery. We provide residential and supported living services and for those whose recovery is well underway our out-reach team will provide support in an individuals’ own home for as long as required.

Support for people with mental health conditions

Glenholme provides support for adults with mental health problems, including those with, learning disabilities, complex needs, forensic histories and addictions. Support is provided on a maximum ratio of two residents per key worker. Residents follow a genuinely person centred care plan or Wellness Recovery Action Plan, in which the concept of ‘choices’ are introduced as early as possible. They have supported access to local services and access to additional therapeutic services such as aromatherapy, music therapy. For those with addiction problems, we offer a fully integrated programme of care, based on an evidence-based model of practice: Integrated Dual Diagnosis Treatment (IDDT). The programme is designed to proactively help individuals recover by offering mental health and substance use services simultaneously, in the same setting, delivered by the same staff, with a consistent and holistic approach. By promoting and delivering a holistic recovery methodology we aim to minimise chaotic lifestyles, offering a supportive inclusive structured day that will focus on positive activities as well as providing practical and emotional support to individuals by way of compassionate listening, motivational encouragement and engagement, and by tailoring person centred recovery support plans that will recognise both the person’s life experiences, as well as areas to focus on and work towards.

Glenholme supports adults with learning difficulties, mental health conditions and complex needs. We operate in a variety of settings, residential, supported living and outreach. We work with those we support, their families, friends and professionals, to provide genuinely person centred support, support which promotes independence and which has the flexibility to respond to changing needs.
